Student Covid vaccine verification form


    COVID information



    For ALL students: Please keep your child home if they have any covid symptoms. Fever, chills, sore throat, headache, cough, trouble breathing, loss of smell/taste. Please get them tested or see a doctor before they return to school. Please contact me if they test positive.


    If your child was tested for covid because they had/have symptoms, please keep them home until you get the results. If positive, please contact me for a return to school date.


    For VACCINATED students: If your child is fully vaccinated (2 weeks after their second dose) they do not have to quarantine if they live with someone that has covid or is a close contact of someone with covid. They should stay home and get tested ONLY if they show symptoms of covid.


    For UNVACCINATED students: If someone at home is sick and getting tested, please keep your child home until that person's test is resulted. Then notify me if it is positive so I can tell you the return to school date. 


    If someone at home tested positive please keep your child home and contact me about the return to school date.

    Immunizations: All students must comply with New Jersey State immunization requirements or they will be excluded from school. Immunizations records are kept on file in the Nurse’s Office for each active student. Parents/Guardians are notified of any deficiencies or required boosters. All new or re-entry students are seen by the school nurse to determine the student’s immunization status and free from any apparent conditions. Please supply the school nurse with annual/updated physicals, any new immunizations and any changes in your child's health status.
